For all other insurances, I am an out of network service provider, meaning that I am not able to accept direct payment from insurance companies.

However, depending on your plan, 60-80% of my fee may be paid to you by your insurance company.

Here are some questions to ask your insurance provider to help determine your out-of-network benefits:

Do I have a deductible that has to be met before I can receive reimbursement? If so, what is it and have I met it yet?

Do I need a written prescription from a physician in order for services to be covered?

Does my policy require a “pre-authorization” for mental health services? If so, is there one on file?

How do I submit a claim for reimbursement?

If you are covered by one of the insurances I take, then I will bill them directly, but you may have a co-pay, depending on your plan.

My self-pay hourly session fee is $159.

Fees are required to be paid at the time of service. Self-pay clients often seek reimbursement from their insurance providers.

As part of the therapy process, a thorough therapeutic assessment with diagnostic impressions and plan for treatment is included.
